> Maryland Health Form (Part 1) - required document for application. This form must be filled out by the parents for each child applying for admission to the GISW. After the application is approved, the GISW health room will send you additional forms to complete before the first day of school.
> & Maryland: Vaccine Requirements 2026/27 for your information, the complete overview of vaccines that are required by law to attend school from preschool to Grade 12 in Maryland, USA. The State of Maryland updates this chart every school year.
- IMPORTANT: Please check your child's immunization records for the type of Meningococcal vaccine they received. Starting in 7th grade, Maryland requires the immunization against A, C, W, and Y which is different from the variant that is usually vaccinated for in Germany. Your child will have to get the correct vaccine to attend 7th grade or above.
- If your child was born on or after January 1st, 2015, Maryland also requires a blood lead test to attend Preschool, School Entry Level, or 1st grade. Please make arrangements for this test before the first day of school.
